We provide a standardised and clinically effective anticoagulation management for patients registered with a Sutton GP who require or receive anti-coagulant therapy at home. Management of these patients is provided by means of a near patient testing (INR testing) within the local community using Computerised Decision Support Software (CDSS).
The key objectives of the service are:
- Provide accessible, safe, standardised and clinically effective anticoagulation management to patients receiving warfarin therapy whilst minimising the risks associated with anticoagulation
- To identify patients receiving warfarin and offer transfer of care from hospital to primary care for domiciliary provision for appropriate patients
- Ensuring the same high quality of service to patients whether accessed in primary or secondary care. Wherever possible, near patient testing is the preferred method, allowing face to face communication between the patient and the health care professional responsible for deciding doses on their test result and any action required as a result
SGPS providing the service means pulling the service provision within Primary Care, closer to home for patients. In addition this has created collaborative working methods with all care providers – GPs, Nurses, HCAs, Acute trust, Community team and Carers alike
