Sutton PCNs Virtual Ward Clinical Chairs

We have an opportunity for Sutton PCNs Virtual Ward Clinical Chairs (3 Posts) to join the team at our GP practice sites in and around Sutton. A great opportunity to work in an innovative and fast expanding Primary Care Networks in Sutton Surrey. Sutton Primary Care Networks (PCN) was formed from the merger of 23 high achieving GP practices and our services are offered from different GP Practices in and around Sutton, Cheam, Wallington and Carshalton. We aim to provide the best quality care for the local community.

The Virtual Ward (VW) opened on 1st February 2021 during the Covid pandemic to try to prevent hospital admission and reduce prolonged hospital stays by providing more intense monitoring of patients at home. The ward started as a Covid VW but now also sees non-Covid patients at risk of hospital admission, such as patients with frailty who need more support or patients having exacerbations of long term conditions such as heart failure, COPD, asthma and infections, where a short period of intense daily review at home, could prevent an admission to hospital. Referrals come from GP practices, community nurses and hospital wards, especially the acute frailty unit and medical admissions unit. Patients can also be stepped up or stepped down from VW into the Pulse Oximetry Service or Complex MDT Service. The VW team is expanding and there is now increasing capacity to take on more patients.

The Sutton PCNs are looking to recruit Clinical Chairs for the VW as it grows in capacity. You will join a team of Clinical Chairs to enable a 1 in 3 month rota. The time commitment will be 5 hours per week but only during the months you are due to cover. Please see the attached Job Summary for further information.

Sutton Primary Care Networks (PCNS) are recruiting PCN Support leads for each of its’ PCNs. The primary focus of the role is to support the PCN Clinical Directors in the delivery of Clinical services across the PCN and Sutton PCNs working collaboratively as well as supporting the development of the PCN generally. The post holder Work closely with other Clinical Directors, Practice Managers, Sutton PCNs Management team and the Training Hub to support the PCNs transformation agenda and meeting the requirements of a number of Direct Enhanced Services (DES’s).

The ideal candidate will demonstrate a wider knowledge and experience of primary care and General Practice with a track record of managing services and workforce in general practice. The ideal candidate will bring strong management experience, and the ability to lead a diverse and dispersed team of people who are passionately committed to the delivery of patient services in PCNs/Practices. We are looking for someone who can earn the respect and support of colleagues and be seen a force for good.

Reporting to the Managing Director, the successful candidate(s) will have proven and significant management experience in health or a related environment, possessing strong interpersonal skills with the ability to build effective relationships with a wide range of internal and external stakeholders. A passion for developing new models of care within Primary Care is essential.
