PCNs CD Population Health Lead(s)

Sutton Primary Care Networks, are recruiting a flexible Clinical Director(s) to work as Population Health lead(s) across the PCNs. The PCNs are currently consolidating after a major period of change and looking to review and refresh all of the services we inherited from the former Sutton GP Federation and to embed the new services that we have launched over the past 6 months. At the same time, we are seeking to ensure that we deliver services of the highest quality that meet and exceed the standards set by the Care Quality Commission (CQC). Working to ensure that the PCNs meet all of its corporate responsibilities.

The successful candidate will demonstrate a wider knowledge and experience of overseeing clinical services which will include;

  • To actively drive PHM transformation through the shared development of delivery plans and optimisation of our resources
  • To support PCN and other provider collaborative developments ensuring that population health needs are central to those plans.
  • Contribute to the delivery of Health Inequalities PCN DES
  • Oversee community strategy / outreach projects
  • Support the assessment of Population health fellows
  • Act as an advice resource for compliments, comments, concerns and complaints
  • Leading on Sutton wide approach to reducing health inequalities

This work will require Board level oversight to support the management team as there is much in the way of policies, systems and processes that needs to be reviewed and revised to ensure our services are safe, patient focused and of a high standard.

Reporting to the Chair of the PCNs Board, the successful candidate(s) will have proven and significant of:

  • being a CD
  • Working at a borough level
  • Service design or delivery or shaping policy/procedures

      This is a part time post of 1 session (4hrs) per week, the salary / hourly rate will be commensurate with candidate’s experience qualifications and experience.

PCN Health Inequality Leads

As part of the NHS Long Term Plan, a main area of focus is the growing awareness of health inequalities. The integration of approaches to reducing health inequalities into local plans and national planning is part of the NHS five year plan from 2019 to 2023/24.

Driving down health inequalities is a central focus of the plan - especially in light of the impact of these inequalities in deprived areas due to Covid-19.

In Sutton, we are working on programmes to reduce health inequalities across our 4 PCNs and are appointing project leads for each PCN programme, to manage these programmes and empower the Health Inequalities teams.

Sutton PCNs are now recruiting for 4 Health Inequality leads to:

  • To manage and develop the Health Inequalities programme.
  • To ensure clarity on objectives and aims at all times.
  • To be an active member of the PCN Health Inequalities leadership group.
  • To support and develop new ideas that progress the programme.
  • To empower the Health Inequalities outreach team members when on site visit.

      We are looking for looking for 4 Health Inequality Leads to work with us in developing our ambitious plans of reducing health Inequalities within our patient population. We are looking for an individual with Health and Wellbeing or population health skills and experience and willingness to develop other skills if you don’t already have them.

      The successful candidate will work under the supervision of a Clinical lead, providing patient care at our PCN site(s).

Extended Access GPs – Sutton PCNs

We have an opportunity for GPs to join the Extended Access team of Sutton PCNs within GP Practices across Sutton Borough including Carshalton, South Cheam, Wallington and Central Sutton. A great opportunity to work in an innovative and fast expanding PCNs within South of London. Sutton has a population of approximately 200000 residents registered to 23 practices and there are currently 4 Primary Care Networks each serving a population of approximately 50,000 patients. Our Sutton PCNs are forward-looking, friendly and focused on providing a wide range of excellent healthcare services to patients in Sutton and the surrounding area. Our PCNs between them are led by 10 PCN Clinical Directors. The PCNs work together as they see the benefits of working together in a larger GP partnership and are delighted to be realising some of those benefits now. Because of our scale, not only are we more resilient and efficient but we are able to invest in continuous quality improvement, enhanced care, new services and training and developing our workforce. We are looking to employ locum GPs to work in our Extended Access
Monday – Friday 6pm – 8pm
Saturdays & Sundays 8am – 8pm
Hourly rate £95 - £110 per hour

Extended Access Practice Nurses – Sutton PCNs

We have an opportunity for a Practice Nurse to join the Extended Access team at our practices in Sutton, Carshalton, Wallington and South Cheam. A great opportunity to work in an innovative and fast expanding PCNs in Sutton. Sutton PCNs were formed from the merger of 23 high achieving GP practices and our services are offered from GP Practices in Wallington; Carshalton; Cheam and Central Sutton.

We are in the process of evaluating the way in which we provide care to our patients. Our business plan includes developing a service that will fit the needs of our patients that are in line with the NHS 5 year forward view and the aspirations set out in “better health for London”.

We are looking for Practice Nurses to work with us in developing our ambitious plans of providing primary health care to our patient population. We are looking for individuals with Primary Care Nursing skills and experience, and willingness to develop other skills if you don’t already have them. The successful candidate will work under the supervision of Senior Practice Nurse and Extended Hub Coordinator.

Working hours in the Extended Access include
Monday – Friday 6pm – 8pm
Saturdays & Sundays 8am – 4pm

We are offering an hourly rate between £30ph and £35ph depending on experience plus NHS Pension and annual leave up to 30 working days + 8 bank holidays (pro rata for part time).
